CIS STUDENTS REQUIRED TO HAVE VACCINES

 
Campbellsville Independent Schools’ students will be required to have some new vaccinations before starting school on Wednesday, Aug. 8. All students are required to have a current certificate of immunization status to attend a public school such as Campbellsville Independent Schools. For the 2018-2019 school year, students in kindergarten through their senior year of high school will be required to have two doses of the Hepatitis A vaccine. Also, students age 16 and older must receive two doses of meningococcal ACWY vaccine to attend school. If the first dose was received at age 16 or older, the second dose is not required.
 
Other immunizations required include:
To attend kindergarten, all children at least 5 years old must have:
Five doses of DTaP or DTP (diphtheria, tetanus, pertussis) or combinations of the two vaccines.
Four doses of IPV or OPV (polio), or combinations of the two vaccines.
Three doses of hepatitis B.
Two doses of MMR (measles, mumps, rubella).
Two doses of varicella (chickenpox), unless a health-care provider states that the child has had a diagnosis of typical varicella disease or verification of a history of varicella disease by a health-care provider or a diagnosis of herpes zoster disease or verification of history of herpes zoster disease by a health-care provider.
 
To attend sixth grade, at age 11 and older, a child must have:
One dose of Tdap (diphtheria, tetanus, pertussis), if it has been at least two years since the administration of the last dose of tetanus-containing vaccine.
Two doses of varicella (chickenpox), unless a health-care provider states that the child has had a diagnosis of typical varicella disease or verification of a history of varicella disease by a health-care provider or a diagnosis or herpes zoster disease or verification of a history of herpes zoster disease by a health-care provider.
One dose of MCV or MPSV (meningococcal vaccine).
